W-9 and Info

All officials working out of Tri-Town Ice Arena are required complete a W-9 tax form in order to receive payment for services. They must also submit their full name, address, SS#, and contact numbers to the referee-in-chief in order to be scheduled.

Once all of the above lessons have been completed all officials must complete the on-line exam and submit it to the referee-in-chief.

Take exam - When you have completed the exam and submitted it for grading, print it out with the results and place it in your student book to review with the mentor/evaluator

Once the exam has been completed please contact the referee-in-chief in order to schedule an observation of a game before working any games at the rink.
